Update: The issue also affects playable Sims, not only NPCs. It seems to be triggered specifically by interactions with the PowerStack Free Weights object. All interactions with this object can cause the outfit change while the Sim is still in the pool.
Additional testing suggests the issue may be linked to the PowerStack Free Weights object and affects both NPCs and playable Sims.
