A clean game is a whole new Sims 4 folder created by the game after you rename the current one in the Documents > Electronic Arts folder. Can you confirm that this is what you have tested and the game is still not functioning?
Or have you moved out your Mods folder only?
Or started with a new Sims 4 folder and moved user data from the old one into the new one?
