Removed tattoos reappear under certain circumstances

I also experienced a similar problem. For me, it was like: if I unselect one tattoo, the green outline surrounding the thumbnail disapears BUT the tattoo on my sim doesn't, I can only use another tattoo so that the previous one can go away.

I also had this issue after removing my mods and clearing the ole cache to test some other bugs.  It happened with a sim I had made before release to be a tattoo artist. Loaded her up, decided to replace all her tattoos with custom ones, only for them to pop back up when I switched the outfit. Removing all tattoos also worked for me in the end, just slightly annoying to have to do.

I did a little experimenting and I found out you can delete individual tattoos, it's just a little complicated. If you have had a tattoo on your Sim while you've been looking at multiple outfits, to delete it you can go to each outfit you've had open and delete the tattoo in each. If you can remember which outfits you've had open, but if you can't just go through all the outfits and delete the tattoo in each, if they have it. If you switch to any outfit without deleting the tattoo from the previous outfit, the tattoo will propagate to the new outfit. Sorry if that sounds a little like I'm repeating myself a little, I just thought I'd try to be as explicit as possible. Again, deleting all tattoos does definitely work if you're okay with starting from scratch.
It seems to me like (and I'm not necessarily knowledgeable here), with the new tattoo system, that when you switch to an outfit, new tattoos are copied to the outfit but the absence of a tattoo isn't, and that's I think what's changed with the new system. Hopefully either the workaround or the more detailed description is helpful.

I also experienced this, I got a sim off the gallery and removed his tattoos. Added a new one I liked. All the old ones reappeared after I switched outfits. 
I use only MCCC and literally 2 CC eyelashes for female sims. 

trying to create a new sim and the randomly generated sim came with a pre-existing tattoo. i cleared it and created new tattoos but then when i switched to the formal outfit category the tattoo i previously cleared reappeared. tried individually clearing it from each category but each time i return to a category i previously cleared, the tattoo reappears alongside the new tattoos. 

attaching images in order starting with the fitness category after clearing it the first time, switching to the sleep category where the tattoo reappears, and then switching back to the fitness category where it shows that the tattoo has returned to that category as well. 

I keep having the problem of when I'm trying to add tattoos to my sim. Random tattoos that I did not add will appear and when I erase them. They reappear when I go to work on another outfit. They don't appear if I erase all tattoos. It seems to happen when 1 or more tattoos are added. Its pretty inconvenient when I want to put tattoos on my sims. 

I've got this happening on Xbox Series X with all packs. I discovered it after my tattooist Sim gave a tattoo to a Sim that I didn't want to have tattoos (Judith Ward). I let her complete the tattoo anyway thinking I could just remove it later in CAS. I could remove it but when I switched CAS categories it came back. I had to remove it again and leave CAS immediately for it to stay removed. Same thing happened with Mila Munch, any further work or browsing in CAS causes the tattoo to return but if I leave CAS immediately the tattoo does not return.
