Every time I get a promotion and the close out of the game the titles revert back to knight and dame. I am now Queen yet am showing as knight and dame.

Just another comment on this topic. I noticed this same bug after I left my sim's lot to work on building another. When I returned to my household, her title had reverted to the previous title.
This is very annoying. I'd rather not have to fix this issue each time I load my household.
