I just got hit with a minor version of this bug. Everything had been going fine, even transfering from the previous head (an elder) to the new head of the Dynasty. Now, I'm not playing in the house of the Dynasty Head for reasons, but I went in to do some maintenance - namely add the recently adopted toddler (via Neighborhood stories) to the dynasty. While I was in there, I re-added the late head of the dynasty as a ghost and when I came out, suddenly I got a notice asking if I wanted to change the romance dynamic between the dynasty head and their spouse to strained.
He now has a bad relationship with several members of the household, and his relationship with the sim I'm playing most of the time took a major hit, though is still positive.
If I were to guess, I'd go with adding either the adopted toddler or the ghost is what triggered it, but that doesn't match up with everyone else's experience.
No mods, PC.
