Children of nobles don't pass on the titles automatically to their children.
First generation noble children receive their titles correctly, but when those children grow up and have children of their own, the next generation does not receive a title.
It happens with both births and adoptions. It also happens both when the parent has title from their parents and/or has joined the Noble career.
The only workaround I've found is to promote/demote them and that gives the children their titles, but it's inconvenient and not how it's expected to work. It becomes an issue with unplayed households too as eventually there ends up being no other noble legacies if you don't have story progression turned on.
