I am having this same issue, but after going back in game and testing the two sims with the glitch, neither one is wearing any nails or accessories. I do have CC and mods installed.
Edit: I found the culprit and it was a skin tone file. If anyone else has this issue and it's not the nails causing your glitch then check for "Lamatisse BARE skintones MERGED". After removing that file the sims went back to normal and I haven't seen this issue since.
