Sims will suddenly go 'Out of the World' to infinity

This happened to me on two separate occasions in two different worlds. I don't know what the correlation was because I had the sims doing different things. The first time the sim just walked right off the map. The second time I was able to stop the sim by resetting him. The teleport cheat works also. I have no mods, but first noticed it after I bought the Horse Ranch and For Rent expansion packs. 

My sim is an elder- and besides grabbing dirty dishes and pouring it into a bake tray to make a new meal- they're pretty okay bug wise.

(strange behavior - for sure a bug, but i feel it adds a funny element to this elder sim so idc.)

However:

 I've caught them walking through walls/walking through fences/walking across debug objects and over entire water features that are not interactable normally to sims --> to the edge of the map where they would have disappeared. (walking in a straight line)

I was able to pause the game and quickly type in [cas.fulleditmode] to Shift -->debug(reset object). before they disappeared into a mountain range and I'd be unable to find them. It did teleport my sim back into their house however I was worried this would be a reoccurring problem for this sim.

I do have images of this saved via [C] to capture this happening - this was in Newcrest, near the twin points(?) smt property.

(by the kids pirate ship playstructure/gyser that's fenced off - there are no fishing locations on this side of the map just sidewalks). 

My sims sometimes disappear in Newcrest. If i focus the camera to them, it seems that they are in the canal/river? But I can’t actually see them, that is just where the camera takes me. Obviously there is no way they could have walked there, they have just glitched down there. I haven’t actually seen them go there, they just sometimes end up there when i’m controlling another sim. They then can’t get out and are just stuck there no matter what.

This has also happened to my sims cat, who seemed to be in an inaccessible bush, and ‘call over’ didn’t do anything. I don’t know if this happens anywhere else, I have only experienced it in Newcrest.

Only loading to the main menu and loading back in to the save seems to fix it, and since i can’t see the sim when it happens, I can’t try resetting the sim.

I have the same problem. Sometimes my sim goes to the bathroom and then they start walking straight out of the world infinitely. To fix it I reset them, but sometimes they appear with the censoring mosaic, so I have to direct them to use the bathroom again to take the mosaic off.  This happened to me at least three times, and the first one was around the Grim Missions before Life and Death came out. 

I play with mods, but I see that this also happened to  people who don't use them, so I hope we got a fix someday. 

I have experienced this issue with two separate saves, my bugged save and my new save file. I play with mods, but I don't think this is a mod issue because it's been happening for months. I find that sometimes pressing shift+click and resetting the objects they're using might bring them back to "earth". Other Sims that interact with that Sim also freeze on the screen, and tasks tend to pile up infinitely. Reloading the screen (i.e., entering and exiting CAS, going to a different lot, etc.) works, but it only works for a Sim that isn't frozen or lost in infinity.

I play on PS5 and no mods/ expansion packs, only the base game. This is the most bizarre gameplay I have ever experienced. When playing, I noticed that one of my Sims was walking through a lake where you cannot go and can only fish in. His task bar was full and going off the screen that the game had set for him to do; yet he wasn't doing anything other than walking continuously in one direction. The Sim continued to walk off of the map, through houses. Since you can't go past the border in normal gameplay mode, I had to use camera mode to check where the Sim was going. His walk was very slow, it became more like a small shuffle towards the end and after 15 painful minutes, the Sim reached the end of the map and then the game went extremely laggy. I am clueless as why this happened and why this Sim wanted to get to the end of the map. If I tried to pan the camera on him, it would just pan to the end of the location that is loaded that you can't get past even though he was no where near there, he was near the edge of the map.

I took a buble bath for my toddler and then ask my sims to give her food in a high chair. Then my adult sims just walk away. She was passant through the wall and floting in the air while walking away. When she finally reach the end of the Maps she just walk in circle and I couldn’t do anything. I couldn’t deleate the list of thing to do for the child or the adult and it was saying that my child was eating and that the mother was giving her food. The child was actually still in the bathroom and couldn’t move. But i quit the game and save my game and when i came back my child was of the chair and full there was a bol on the chair and her parent was at the front of the house like of she just came back from work.

Happened to my cats. Ps5. They just started walking through the water in sulani and then disappeared when reached the side of the map.
