This also can happen if you own Enchanted by Nature and have the luck mechanic on. When your sim have bad luck, it will result in negative drop in relationships at every conversation.
on every conversation? That sounds like a terrible game mechanic because luck is just randomized.
From what I've seen in my game, about 90% times it will result in fail. You can influence luck with charms and potions, but yes, I find it to be somewhat excessive.
