I too am having the same issue with no cc or mods in my game. I'm on windows 10. For me, it happens in game. When I had Anya in CAS before I started the game from day 1, I didn't see any hair glitch, but then it happened the moment I went in game with her. It continues to happen. :( Here's the zoomed in and out pictures.
