I have the same issue and hope it gets resolved😰
Rory and none of the Volkovs have the glitch. The librarian npc has the glitch (see attachment) and one of the sims Rory gave the cursed bite to in game glitched (see attachment of Sachiko Imai). Another sim Rory bit in game hasn't glitched so I don't know if it's random (see attachment of Louis Higginbotham). Both of these sims were generated by the game itself. Also, the game let these two sims join the Wildfangs pack as HUMANS which shouldn't be possible, but that's another bug which may or may not be related.

How to encounter the glitch: Transform a sim into a werewolf in game via cursed bite or just go the library and the werewolf librarian might be glitched if it's a game generated npc, like my librarian is (she replaced the original Wolfgang Wilder, who is still alive in my game but for some reason isn't the librarian anymore).
When did it happen: I haven't actively played with werewolves since a year ago and only encountered the bug this month when I started playing again (August 2025).
I hope it gets fixed too! But I found a work around, click on that werewolf and go to cheat fulleditmode, unlink and swap the werewolf head to whatever, click back to human form and save. It will change back,not a fix but this helps.
Thanks! 😁 Switching to a premade werewolf face fixed the issue! Still hope EA will fix this bug in general.
