@Murry2211 I think you would need a mod for that. I suppose you could save as soon as you go into labor, and if you find you have twins you could exit without saving, then re-enter and do the birth again and use the twin names.

@Murry2211 If the game decides to play stubborn and will only give you a single baby instead of the twins you are hoping for, there is still another way for you to get the twins you want by using the cheat called cas.fulleditmode. You'd have to wait till your baby is a toddler because unfortunately the game doesn't really fully recognize infants as "Sims" until they reach toddler age. This is not a mod so it will work on PC, Mac, or console.
As soon as your baby ages-up to being a toddler, open a cheat window and then type testingcheats on and press Enter, then type cas.fulleditmode and press enter again. Exit the cheat window. Then do the following steps:
- Have one of your adults click on a dresser or mirror (like you would for planning outfits) and select Change Sim.
- With the toddler selected, click the Add a Sim button in the lower left of the screen and Play with Genetics from the pop-up menu.
- In the Create His/Her: block, choose Sibling & the game will generate a brother or sister (your choice) for the selected Sim.
- Click the Randomize Twin button, and congratulations you now have twins! :D
The game will prompt you to choose a name and trait for your new toddler as well as any outfits, hairstyles, etc., you choose. You should not have to edit their appearances any beyond that, they should be identical twins for appearance.
I tested this method with the Bheeda family from the City Living EP. I created a twin sister for their daughter Cassandra and then aged them up to Teens just to verify they would still have the same appearance as they grew older. It might not have been 100% perfect but it was pretty close! I could not tell them apart.
Btw- the cas.fulleditmode cheat can be used at any point in your game to change things like names, traits, appearance, etc., for any of your Sims.

@Murry2211 There was another cheat called pregnancy.force_offspring_count that you might still come across in various game guides when researching this topic (i.e. multiple babies). It used to allow a player to force a specific number of babies for a pregnant Sim. This cheat was disabled by EA about a year or so back because it had the potential of REALLY messing up your game save. If you see that cheat still listed anywhere just ignore it.
@Murry2211, in addition to what @SheriGR and @RandomBuzziness have suggested, you can also try Reward Traits and Lot Traits to increase your chances of having more than one baby. The Lot Trait is free and is called "On a Ley Line" and increases the chance of having a baby, and supposedly increases the chance of multiples.😇
But for multiples it would probably best to get a Reward Trait from the Aspiration Rewards Store on your Sim's Aspiration Panel. There are a lot to choose from, the one that increases the chance of having twins or even triplets is the "Fertile" reward trait.🙂 To be extra sure you can have both the mother and the father get this trait.
However, there is a Bug Report on the Fertility reward trait. 😞 There isn't a lot of information on if this is an actual bug or not, as only a few people have reported it.🤔 Here is the thread on it https://answers.ea.com/t5/Bug-Reports/NEEDS-INPUT-Fertile-Trait-not-working/td-p/8632454
