@ernescig  A bit more detail is needed to be able to help solve your issue. What happens when it is time for your doctor to go to work?
Are you on PC/Mac or console?

What packs do you have installed?

What is your game version (that will confirm which patch was last installed).

Do you have any mods or cc (custom content) installed?

Try removing (not disabling) mods / cc and delete the localthumbcache.package and any other cache files.

Testing on a vanilla (no mods/cc) folder is the best way to investigate causes of issues.
A vanilla folder is the the new folder generated when starting the game for the first time.

It is found at \Documents\Electronic Arts\The Sims 4 and contains all your current game saves and other information.

    He just goes to work (doctor) as to normal works.

    PC

    Island Living, Get to work, City Living, Jungle Adventure and one accesories pack (Washing Clothes)

    The newest one (I always update the game)

    Yes - Tons of CC and some mods

    @ernescig 

    Mods and custom content (CC) can be fun to use but unfortunately they cause unexpected results.
    They need to be updated by the creators to continue to play nice with the game.
    It would be a good idea to test on a new save with a clean folder.
    The simplest way is to do the following steps:
    Rename your The Sims4 folder in Documents\ElectronicArts to Sims4_Backup
    That will preserve all your saves and content so nothing is lost.
    Start the game and a new save to test the career. If the career works correctly you will need to test yor mods to see which one is causing the problem.
    When adding/testing/deleting mods and/or cc it is important to delete the localthumbcache.package and any other cache files.
    Using the 50/50 method is suggested to speed up the process.

    @ernescig You will not find the hospital on any map as it is a hidden location. Only Sims in the Doctor career or pregnant Sims going to give birth have access to it. With your Doctor on the home lot you you will get a choice to send alone or follow them to work. By following them you will see the lot.
    Did you test on a game with a new folder?
    Move the entire folder found in \Documents\Electronic Arts\The Sims 4 and let the game generate new fresh files.
    Do not put any of the old content back into the folders.
    Create a Sim and test the career.

    @ernescig Remove all your mods, ALL of them. Then start a new game (so you don't mess up your old one). Start the career again with a new sim.

    If it works fine, then you have a mod interfering. You'll then just have to bite-the-bullet and go through them all and figure out which one.
