I did confirm that I was on the correct account - it was connected to my steam. I was initially on the wrong account but switched to the correct one, and it still showed no issues, bans, or anything of the sort, with no option to appeal anything.
I tried the form and there is no option to email.
I ran into the same problem, and the web form does not have a submit button.

Just to confirm, are you seeing anything on your penalty history page? This page has more information on bans, and how to appeal one you feel is unfair.
If nothing appears on that penalty history page, please confirm that you're on the right account by checking the account's purchase history, connected accounts, and time played. Typically, when the penalty history tool is unpopulated, it's due to being signed in to the wrong account, which happens more often than you think.
Once you've confirmed you're on the right account and still can't see anything in your penalty history page, please send me a screenshot of your connected accounts and penalty history page via a direct message. We can continue from there, thanks!
