Game runs terrible, even capped at 190 or180 etc.

I have a rtx 3080, with a 5950x alongside a 240hz monitor. If left uncapped (240fps), game stutters+tears. If capped at 190, game runs okay BUT periodically, and espicially in some areas like solar array on olympus, and fragment on worlds edge, the game is unplayable. Its not as much stuttering as it FEELS like fps is dropping by 100, even though it is not. Or another example, is if my mouse polling rate falls to 250 somehow. However, neither of these two are happening, it just feels like it. It becomes nearly impossible to aim and feels like the entire game just slows down entirely. Ive been trying for over a year looking for a fix, resetting windows, buying new hardware for my PC as well as new mouse(s) and keyboards. Nothing fixes it. This feeling of "slowing down" occurs on ANY fps I try. Lowering to 144hz 144fps just causes TERRIBLE tearing and stuttering. PLEASE HELP. 
Some troubleshooting ive tried:

- checking high temps (all fine)

- benchmarks (come back fine)

- Swapping hardware 

- resetting windows 

- Trying different monitor/cables

- Windows 11/10 or even older versions XD

and a lot more that aren't coming to mind at the moment.
