Having all four of those connection symbols showing up certainly isn't normal, so if you've already gone through the basic troubleshooting steps you mentioned, we'd definitely want to try and see what's going on there.
Are you able to grab and attach a UOTrace file so we can get a better look at the route your connection's taking to the servers?

So these are definitely showing quite a large amount of packet loss. Just based on this your home network connection (router) is fine, which is likely what your ISP looked at to tell you everything seemed ok.
Just a couple hops down from that however, we do see some extreme packet loss in multiple locations. This is well before your connection's even reaching our servers which unfortunately means it's not something we have any control over. Reaching back out to your ISP to speak with someone that can read a traceroute or pathping file and showing them this info will be the best way to go.
While much of this is likely outside your ISP's direct network as well, they'd need to chase this down on their end to get in touch with the providers of those hops having issues.
The only other steps we could really suggest in the meantime would be any of these troubleshooting steps you haven't already tried. In particular flushing and changing your DNS would be the things I'd look at if you get the chance.
