My friend's account got reset for absolutely no reason

I don't how but my friend's account got reset, when he launched apex it asked him to change his password and when he did all his cosmetics and  progress were reset he tried to reinstall the game 2 time and it always was bugged how can hd solve this ? And he's on Ps4
