I went through the exact same issue. I got my account back but it took 15 forms over 4 weeks. The process was awful but I pushed support often for any other options and found that this is definitely the only way. Try providing proof of purchase of EA products exactly as outlined here: https://help.ea.com/en-us/help/account/how-to-provide-proof-of-purchase/
I also provided an extremely extensive list, and I mean extremely extensive of my Apex Legends unlocks ie. skins/all cosmetics. To do this I made a new account, logged into Apex, scrolled through all the skins and noted from memory which ones I had on my main account.
I also provided my billing addresses, the name of my ISP and any other ISP I've been a customer of while playing in the field where they ask for your IP address.
On top of this, I listed every single EA game I've played on the account, going back 9 years. For physical games I followed the proof of purchase guidelines above if I had the receipt lying around but otherwise I provided the long number printed on the disc.
I also provided a pic of my ID showing that the name matches the one on the account.
