Unable to Submit Ban Appeal – "Case Not Created" Error

I have tried submitting an appeal through Penalty History more than 10 times, but I always get the "Case not created" error. I also tried in incognito mode, but the result was the same.

I then tried using the web form, but there is no "Email us" option to submit it.

I've attached screenshots of both issues. How can I submit my appeal to the Terms of Service team?
