Reproducible graphical issues with 32:9 displays
During the final missions of the main story, if you are playing the game in a 32:9 resolution (5120x1440), geometry and textures completely fall apart. Models will blink in and out wildly, textures wont load, Rook becomes floating hair... its nightmare fuel. This will happen regardless of the "Cinematics Aspect Ratio" setting being enabled or not.
I've reproduced this issue on several different PCs running different hardware. The work around I've found is to set Windows resolution to a 16:9 aspect (2560x1440) to eliminate the issue.
