Cant manage to create a ticket(case)

Im having trouble with input lag and game crashing out of nothing while i have good ping im in europe i got like 18-20ms. I want to open a case in the EA help in order to send my issue. I fill out that form and only have theese options of self support. I want to talk to someone in the company.
