Crashing on PC + controller issue

Dear EA Team,

I would like to report a bug which I'm experiencing from day 1 of early acces, meaning it's almost 2 weeks now - TWO WEEKS EA!!!

Since 18th of Sep  I'm constantly being kick off from the game (like hitting alt+f4 combination), without getting any notification. Have tried many, many things described by other users, nothing works.

Additionaly I have controller issue which is not working in Rivals. 
Due to the fact that I'm not able to play Squad Battles I cannot unlock trade market, meaning I can do basically nothing in the game.

PC: meeting min requeirements, windows10
Platform: PC through EA app
Summary of the issue: I'm constantly being kick off from the game (like hitting alt+f4 combination), without getting any notification. The only playable game mode is Rush.
When exactly does the game crash? Couple seconds after starting any match type (Moments, Squad Battles)
Recent timestamp (date, time, and timezone): since 18th of Sep till now

I have bought Ultimate editions which supposed to provide me an early access. Please refund me the money spend on a broken game or fix it and compensate in the game.
