pc crashing

Hi

i bought the game from steam app  4 months ago and the game was running .Sometimes appeared an extreme input delay but after i closed it and opened it again ,was fine. 

For the past 2 weeks i cant play the game.After less than 5 minutes (sometimes even seconds) its freezing completely , and sometimes if it lets me  enter to ultimate team and put me in a game , after seconds its freezing . The freeze is not coming off until i close the app with alt + f4 or by the pwer button...I have done everything...heres is my DxDiag report.
