PC lag and start-up crashes

-Corner flags were only twice in World Tour 4 days ago, looked like a giant blue/silver cup trophy except oversized, bigger than the players in height and width.
-I've tried auto, low, medium, and custom medium resolution with everything else off. Mainly when players come on-screen (clearance) or shift between sprinting fast on attack/press, the game literally slows down, then some players move super fast and catch up. Connection monitor stays stable. Task Manager shows less then 50% cpu, less than 10GB ram, and less than 50% gpu pretty much all the time. This lag/stuttering is inconsistent. Only mode it doesn't occur in is Rush.
-If steam doesn't see/recognize the controller before starting the game, the game sometimes picks the controller up. But I have to alt+tab or click the app on the taskbar EVERY time it loads, because the game doesn't command focus after the splash screen. So it loads as keyboard first, then I have to press a button so it sees the controller. But as long as steam and the PS accessories app see the controller before the game loads, I don't seem to have a problem (cross fingers). It's when i turn on the controller during or after the game loads, the double input bug happens.
