Unable to enter Rivals or Champions matches – disconnects after lineup screen

Every time I try to play an online match in Ultimate Team (both Rivals and Champions), the game successfully finds an opponent and begins loading into the match. I can see the pre-match lineup/introduction screen showing both teams, but immediately afterward I receive the message
