POTM didn't claimed

Hi EA team  am suffocating with this i tried a lot to attach all required players for claiming POTM but it didn't allow me claim or to give option of exchange button why's EA !?.

    You need to scroll down, as most commonly players think that POTM has 3 requirements, whilst it has 5.
    If you have fulfilled all 5 requirements, then post the screenies please?
