While the game does still require picking either a male or female base frame, a lot of what you describe is already possible. Clothing style preference, ability to get pregnant or get others pregnant (or neither), ability to breastfeed, and using the toilet standing can all be set independently of the physical frame. Plus you can set they/them or even custom pronouns - in English, at least. I'm not sure if that ever released to other languages.
In fact, Zain Amir, a townie in Businesses & Hobbies, uses they/them pronouns and can neither get pregnant nor get others pregnant. They have a feminine frame & clothes, and I think I've seen them use the toilet standing.
