No Grim Reaper Bug?

Hi all, like 20 mins ago one of my child sims drowned, i had no notification and the grim reaper didnt show up at all. I only noticed when i saw that he was taken out of my household. Is this a new bug or is there something I potentially missed? Kinda shocked bc normally i have to try to kill sims but this was just random lol. Im pretty sure he wasnt tired but there was a gathering in the pool so maybe it was too overcrowded? idk lol. I also went into build mode about the same time i think it happened bc once i exited it he was gone and just his ghost in the pool, followed quickly by an urn and him disappearing.

ETA: im not playing with any gameplay mods, I still havent re downloaded them since the patch
