Sim being removed from household + deleted

I created a household with 2 sims in a new save file, male and female. I saved and exited CAS and moved them onto a lot, when I loaded into the lot, the male sim had disappeared from the game entirely like he was never created. I saved and exited the game, re-entered and saw the sim was still seemingly removed entirely. I exited to manage worlds to edit the household, recreated the male sim once more and added him to the original household. I loaded onto the lot, the male sim was present but not at the bottom of my screen as part of my household (MCC command center said he was in the household however). I saved and exited the game once more, removed all mods, re-entered and the sim was once again completely gone like I never created him. I tried again without mods and the same thing persisted, if added in CAS through manage household, he is on the lot but not in the household and is deleted if i exit the game

Any ideas what is happening?
