Was she in her own household, or living with the children and the father?
If she was in her own household and it wasn't favourited then she was probably culled.

@Kris152004Aw man.
"Culled" is... basically deleted. The game has a limit of the number of Sims it can have (it's an adjustable number in settings) and if you're getting close to the limit it deletes some unplayed Sims. This is done for performance reasons, and they recommend you keep the number low if you don't have the latest and greatest computer.
