My sims are walking into the distance and escaping my clutches

When my sims autonomously needed to use the bathroom they would just walk into the distance and leave the map??? Are they escaping the sims 4 and attaining free will? This happened to my grandma and dad sim so idk what the issue is… going to manage worlds and coming back seemed to work. Also my sims are blurred when not naked? And that portrait from life and death just showed like a really stretched out face of my babysitter for some reason… like…
