What surface is your Mac on when you play? I had the 16" M3 Max and don't remember hearing the fans even in live mode but admittedly I don't spend much time in live mode. I always have the laptop on a USB cooling fan pad which helps cool the bottom casing where a lot of the heat is designed to dissipate.
You don't want to quieten the fans though. They are cooling your Mac to stop any potential hardware damage due to overheating. The settings you will need to change in-game will be lighting, reflections and edge smoothing. Toggle the laptop mode on and see if that makes a difference. Try windowed mode versus full screen.
The number of sims on a Lot, the actions those sims are doing, the size of the lot, the amount of objects to render on the lot, weather settings if you have Seasons, Mods and CC, all contribute to the amount of power your game requires to run smoothly. You could test by building a very simple small house, a box effectively, with one sim and see how the fans kick in in live mode.
Also make sure you have nothing else running when you're playing. This includes apps running in the background such as Spotify, 3rd party file syncing such as Dropbox/Google, Adobe, anything that's showing as running in the background and will typically have an icon in the top right corner of the menu bar, apart from the usual power, time etc.
