PC keeps freezing after the initial sims 4 loading screen

So, hi for the past few months I haven't been able to play sims 4 because my PC keeps freezing after the initial loading screen. If somebody could help me it would be nice. Here is my DxDiag

Hey so it turns out its not my pc which is freezing, its my monitor. Now I have tried using my TV as a secondary display device and the game haven't caused my PC to freeze until now . Note that my TV is connected to my PC with an HDMI cable however, my monitor isn't it using the display port. But I have previously played sims 4 for hours using my monitor so I really dont understand why the game is now causing my monitor to freeze. Any help on this matter will be greatly appreciated.

    Also note that the HDMI port on my monitor is broken so I am kinda forced to use the display port.

      Also it seems like when I am using my monitor as my primary display the pc keeps freezing. Could it be a driver issue or something???
