Fix #1: Change your squad + remove ALL evo players
This is the most successful method.
Steps:
1.Create a brand new empty squad.
2.Do NOT include any evolved or Evo-eligible players.
3.Set this as your active squad.
4.Go back to UT → New Items → try redeem again.
This forces the server to refresh your player database.

These are players who have the blue “EVO” badge on their card and already completed an evolution.
Examples:
Your evolved winger
Your evolved defender
Any upgraded Evo player you have used before
Do NOT put any of these in the new squad.

Players who can still be placed into an evolution (Evo‑eligible players)
These are players that:
Match an active Evo template
Can still be evolved
The game recognizes as “eligible for an Evo”
Usually these are bronze, silver, or low-rated gold players that match the evolution requirements. Even if they are NOT evolved yet, if they can fit into an Evo, avoid adding them to the squad.

Fix #3: Log into the Web/Companion App → Club → Clear Unassigned Items
You already tried checking the Companion App, but do this specifically:
Log in from mobile or web
Go to Store → Unassigned
Try moving the item manually
If it errors out → good (that means it's the same bug)
Close app → restart console → open UT
Hopefully, this will help you. That was what the EA TEAM provided to me, but it still does not work for me.
