doesn't open CAS to actually customise a tattoo, just starts tattooing the sim without even choosing a design

did you never have mods/cc or remove them ? If you removed them, how did you remove them ? Disable or move to the desktop and delete the localthumbcache file ?
Did you already try a game repair ?

did you wait till the tattoo was done ? For me the window popped up at the end.

Having the same problem here, even when I wait until the end of the tattooing action, nothing happens. The sims just get off of the table and tell me how awesome/terrible their tattoo is
Update: Fully removed mods folder and the localthumbcache file and still having the same problem. This is ONLY happening with male sims, all of the female ones are working fine
