I have this problem too. when I visit the clinic there are no staff, the vet doesn't come. I have no mods. I reinstalled the game. nothing helped.
in both the original version and the one downloaded from the gallery, the clinic is without employees. Always, in my saves and when I start new game.
Did you download the latest patch? Unowned shops and vet clinics work normally for me again (vanilla on PC without B&H).
