Sims will suddenly go 'Out of the World' to infinity

Hey so Im on PlayStation 4 and was making a world last night and the command bar wouldn't go away so I stopped playing for the night and just got back on and it was working again but then I tried feeding my toddler and my sim walked through the wall down to the ground level (was playing in San Myshuno) and just kept walking I couldn't stop her I tried cancelling all her actions and she just kept walking and ended up completely off the map although her actions were like she was still in the apartment like normal even though she was off the map. Had to just close out sims to fix it couldn't even save because a fire started while she was walking. I'm tired of sims 4 on console always being bugged.
