Sims will suddenly go 'Out of the World' to infinity

Hey so Im on PlayStation 4 and was making a world last night and the command bar wouldn't go away so I stopped playing for the night and just got back on and it was working again but then I tried feeding my toddler and my sim walked through the wall down to the ground level (was playing in San Myshuno) and just kept walking I couldn't stop her I tried cancelling all her actions and she just kept walking and ended up completely off the map although her actions were like she was still in the apartment like normal even though she was off the map. Had to just close out sims to fix it couldn't even save because a fire started while she was walking. I'm tired of sims 4 on console always being bugged.

I'm on pc and I have the same issues, It's so frustrating! I just want to play the game, but these bugs make it impossible to do so.

I also experienced the bug recently twice with two different Sims. I don't play with mods.

I will wake my Sims up to use the bathroom and they'll suddenly begin to walk through furniture, walls, and other houses. Restarting them is the only way to stop them, and then they have the pixel censor on their lower half. I'll prompt them to use the bathroom again and it will disappear. It's like they're sleepwalking. This has happened to me twice already since the October 2, 2025 update.

I had this bug for a while and I thought that it was mod related, but I am playing playing without mods or CC since the September patch and I just had this issue when my sim went to the toilet. I restarted her with the testingcheats menu and had the mosaic censor that disappears when I ask her again to use the bathroom. The only difference is that the walk she does it's not laggy as before removing the CC.

The first time I remember happening this was before Life and Death expansion pack, but after the patch of the Grim Reaper missions
