Whenever my Sims interact with Sims outside my household, all social interactions cause a drop in both friendship and romance levels, instead of increasing or staying the same.
This issue occurs consistently in every save and I have no mods or custom content enabled.
I have tried restarting the game and clearing the cache, but the problem persists and negatively affects normal relationship gameplay.

If you have fears turned on in your settings, check if your sims, or the sims you are talking to have a fear. This sounds a bit like Fear of Being Cheated on or Fear of Being Judged.
This also can happen if you own Enchanted by Nature and have the luck mechanic on. When your sim have bad luck, it will result in negative drop in relationships at every conversation.
Yes, all interactions with other sims are negative and the relationship worsens. I play on PC, I think the problem started since I bought Enchanted by Nature.
