Frequently sims will talk to another sim or group of sims but instead of facing them, they will talk over their shoulder. Very annoying and hard to tell who is actually being spoken to. This even happens if an adult is talking to an infant/toddler.

When sims are chatting, they are talking to other sims backs. They wont face each other. This has been happening since update before Enchanted by nature.
Could you please provide the following information
- Are you using mods/cc in your game ? Please remove them and test without them if you do.
- What platform are you playing ?
and if one of them moves they'll usually keep talking to where the person was previously standing for a while

Yes, this is an issue that goes a ways back. Just does not seem realistic to hold conversation with sims when they turn around and look over their should to talk to your sim. I didn't use any Mods when this happened.
This happens to me all the time.
