I have 2 files for you. In order to produce the file, I list the steps I took.
- I started a completely new save file with no mods and no cheats.
- I created 2 teenagers in CAS and moved them into Willow Creek.
- I aged up 1 of the teenagers to young adult and got the 3rd trait. Graduation is now set for Sunday at 6pm.
- I kept the other teen in the house and just triple sped to Sunday and the young adult had a job from 8am to 4pm to make it go faster.
- At 4pm on Sunday, I placed a cake on the table and created SAVE ONE: Slot_00000008.save
- If you age up the remaining teenage sim at this point, there is no bug. She gets her 3rd trait.
- I restarted the game at the same point as #5 and did NOT age up the teenager this time.
- At 6pm, I went to graduation with the teen and the young adult.
- The young adult graduated and had her graduation hat on the entire time. She threw it in the air at the end, but it stayed on for some reason even after throwing it.
- I waited until the ceremony completely ended and everyone left. It was Monday morning already.
- I went home with the teen and the young adult, but the young adult was still wearing the graduation cap even when showering.
- I placed a cake on the table with candles and created SAVE TWO: Slot_0000000a.save
- If at this point, you age up the remaining teenager in the house, she will not get a 3rd trait or graduate.
I have a save from before the bug and a save after the bug with steps on how to recreate the issue. Any teenager you add to the game at this point and age up with a cake while in high school will have the bug. It is a stuck graduation event as I said before. The event is marked as in progress and never completes. I couldn't figure out how to get rid of that graduation hat no matter how many times I changed the young adult in CAS.

I don't want them. EA wants them. Best is to DM them to a Sims Community Manager.
I sent the info to EA_Cade. Thanks for editing the post. I tried to post them as links before, but the forum keeps giving me random errors. It complained about an invalid span or something when I had them as links, so I just put them as plain text. The rich text editor is buggy with Chrome. I've had issues with other posts too.
