Submit Appeal Leads to Support Options Page

Hi EA team/mods,
My Apex account is permanently disabled: "violated User Agreement". Penalty History empty, no ban email, haven't played >2 years (~early 2024)

When submitting appeal via help.ea.com (on mobile/PC, main account and new temp account), it goes to "Your EA Help support options" page. My text appears but no new case in My Cases, no confirmation email/case number.

Happens the same on alt account. Old cases from 2024 blank/closed. Please escalate to TOS team or fix the bug so I can appeal properly.
