Frequent Reboots while playing the game

It has started since season 4 mid season. Whenever I am playing a game mid match the game crashes and reboots my system. This happens at random but mostly in-game. If i am lucky I might be able to complete a match. Attaching Dxdiag. This is really frustrating. And same issue has carried forward to season 5. I had tried disabling exploit protection, limiting framerates, lowering graphics settings but still issue persists.
