Clubs, an error has occurred, multiple players being kicked every match

Playing clubs with two friends tonight, for 5 matches in a row we have been receiving “an error has occurred” messages and losing two or more players, this forfeiting the match.

Seems to occur every time we score a goal!

Has anybody else experienced this issue?

Upon further inspection, this appears to be related to the formation we were using (451 Flat).
We have played using other formations and Archetypes to test out, and the only time we get errors are when using the 451 formation!
