Broken heart - Unable to offer Death Flower

Hi. Is a broken heart considered a natural cause? In my game, the living bystander is unable to "plead" as well as not be able to offer the death flower. There is only the afterlife question regarding the deceased. (I do not have the expansion pack "Life and Death", but I do have "Lovestruck".)  Thank you.
