Yes, only one Sim living in the house, he has since moved in with GF but he was living alone when it happened and he didn't have any guests over, just him and the chef. I tried doing it again before moving him to see if it was a one time thing, but the same thing happened, ordered Glacier Iceberg Lettuce, charged for double and given two of that one dish.
