You can play the 'base game' without any expansions, which includes a great deal of content and everything you 'need' to play the game. The expansions simply add extra functions and content like additional 'worlds' (towns), additional items (like household items, building items, clothing, and sometimes a career or two). For instance, the 'Seasons' expansion will add the 4 seasons to the game, plus a bunch of items and interactions that make use of the seasons, and even a couple of new careers. Each expansion has it's own features and items it adds to the base game... but nothing that is essential to playing the core game.
What is it like? It's a bit like digitally playing house for adults on steroids. It depends on what you like and if it is your thing. I love it and have a lot of fun building buildings, developing households of Sims, challenging them to excel and advance in their careers and maintain their household well, plus interacting with the community and a lot of different aspects of their world.
