I've seen this issue reported by someone else. Whole City Living buildings were suddenly appearing in other worlds. To my knowledge, there was no way to fix it.
When you say it happens with and without mods, you mean you started a new save with no mods/cc and it happened again or did the problem just not disappear when you removed your mods ?
