Sim CAS glitch.
He just was walking and put on every outfit…. I don’t even have him wearing any of these clothes he has on. Hair and facial hair glitched too. It put the skin details on a shirt.. eeww

Have you tried going to CAS and back or travel somewhere ?
If you're not using achievements on this save, you can also try resetting him.
