Please move your whole Sims 4 folder to the desktop then launch a brand new clean game. Do not copy over any saves or mods. How is it?

I have done that but still have the same issue.
I previously have also repaired the game through origin, made sure origin is disabled in-game, I've uninstalled and reinstalled both origin and Sims 4, I even restored my MacBook to factory settings incase it was the problem.
I don't use any mods or cc if that helps also.

Could you post a screenshot of your in-game graphics options? Could you set up a new user account on your Mac and see if it happens in that account?- 6 years ago

Do you have anything running that could be conflicting with the game? Any apps launching on startup like monitoring software, antivirus, streaming? Could you also post a screenshot of the speckled screen you describe?
